The departure gate for the flight to Rome was a chaotic symphony of human anxiety.

John felt a familiar tightness in his chest, the mild, predictable pang of his diagnosed heart condition, exacerbated by the stress of travel. He reached into his pocket for his nitroglycerin pills. “Here, honey, let me,” his wife, Karen, said, her voice a little too loud, a little too solicitous.

She intercepted his hand, producing a different, small pill from her own purse. “This is that new, fast-acting one the doctor recommended. He said it’s much better.”

John hesitated.

“I don’t remember that.”

“Oh, you and your memory,” she chided playfully, her smile bright and brittle. “Just take it. I even brought you some water.” She twisted the cap off a bottle of water and handed it to him.

He was tired, the stress of the airport pressing in. He trusted her. He swallowed the pill and took a long drink of water.

The effect was not what he expected. The tightness in his chest didn’t recede. Instead, a strange, prickling itch began at the back of his throat.

His breath hitched. He felt a sudden, alarming warmth spreading across his face and neck. “Karen,” he gasped, his voice already sounding foreign, constricted.

“Something’s… wrong.”

“It’s just the medicine working, darling,” she said, patting his hand. But her eyes, he noticed through his swimming vision, were not filled with concern. They were wide, watchful, and terrifyingly cold.

He tried to stand, a wave of dizziness washing over him. His throat was closing. It felt as though he was trying to breathe through a straw.

Red, blotchy hives began to erupt on his neck. He clawed at his collar, his breath coming in ragged, whistling gasps. It was then that Karen began to scream, and her performance began.

“Help! Somebody help! It’s his heart!

My husband is having a heart attack!”

Paramedic Martinez hit the ground running. The call had come in as a “cardiac arrest, male, mid-fifties,” one of the most common and urgent calls they received at the airport. He and his team pushed through the circle of gawking onlookers to the scene: a man on the floor, his face swollen and alarmingly blue, and a woman kneeling beside him, hysterical.

“It’s his heart! He has a condition!” Karen shrieked as Martinez knelt beside John. “I gave him his heart pill, but it’s not working!”

Martinez’s mind was a flurry of clinical calculations, his eyes scanning John’s body for data.

Severe cyanosis, yes. Labored, stridorous breathing. But… his eyes locked onto the angry red welts blooming across the man’s neck and face.

Urticaria. He saw the pronounced swelling around the eyes and lips. Angioedema.

This wasn’t the classic presentation of a myocardial infarction. The puzzle pieces weren’t fitting. As he prepared his equipment, John’s eyes fluttered open.
